If newly applying please fill form no. 6, if transferring from any
where to any where then please fill form no. 8A. Down load the forms

of application is 16th April. You need only two documents :
Submit the forms at the nearest City municipal Council Office,
Bangalore Mahanagar Palike offices. Find attached the addresses.

proof you need to bring in the Xerox of following documents along
with original for verification. Original will be returned right on
hand after verification.
1. Date of Birth Proof : any one of either Matric Certificate /
Passport / PAN Card.
2. Address proof : Any one of Rental agreement / Passport / PAN Card
/ Address proof from employer on office letter head, seal and
signature / Address proof from a nationalized bank on letter head,
seal and signature.
3. ID Proof : Any one of Passport / Driving License / PAN Card /
existing election ID card from other state.
